EMERGENCY MEDICAL AUTHORIZATION: I authorize Memorial Baptist Church leaders to obtain emergency medical treatment for my child should illness or injury occur and I cannot be reached. Every reasonable effort will be made to contact me before treatment is obtained.

TRANSPORTATION PERMISSION: I understand that campers will travel to scheduled afternoon activities during MBC Day Camp. I give permission for my child to be transported by approved Memorial Baptist Church drivers in church-owned or approved personal vehicles. In the event of a medical emergency while off campus, I authorize Memorial Baptist Church leaders to obtain emergency medical treatment while every reasonable effort is made to contact me.

LIABILITY RELEASE: I voluntarily release Memorial Baptist Church, its pastors, employees, volunteers, and leaders from liability for injuries arising from ordinary risks associated with participation in Day Camp, except in cases of gross negligence or willful misconduct.

PARTICIPATION ACKNOWLEDGEMENT: I understand my child will participate in Bible lessons, recreation, indoor and outdoor activities, and scheduled off-campus activities. I understand participation involves ordinary risks associated with these activities.

BEHAVIOR EXPECTATIONS: I understand campers are expected to respect leaders and other campers, follow safety instructions, and demonstrate Christlike behavior. If my child's behavior becomes unsafe or consistently disruptive, I understand I may be contacted and asked to pick up my child.

YES, I give permission for Memorial Baptist Church to photograph and/or video my child for church publications, social media, promotional materials, website, and future ministry use. NO, I do not give permission.

(Memorial Baptist Church may request photo identification before releasing a child to an individual who is not known to our staff or volunteers.)

Time: 9:00 AM–4:00 PM
Cost: $50 per camper
Includes: Lunch, snacks, and a camp T-shirt
